I-CARE4OLD – AI-Powered Decision Support for Complex Elderly Care

I-CARE4OLD is an EU-funded digital health platform designed to transform how healthcare professionals care for older adults with chronic complex conditions (CCC). Built on over 30 years of real-world medical data from eight countries, the platform uses advanced algorithms to analyze massive datasets and deliver individualized care insights.

We contributed to the development of a system that:

• Leverages AI to identify disease clusters and health trajectories

• Analyzes lifestyle, behavioral, clinical, and psychosocial data to support personalized treatment strategies

• Integrates InterRAI data from countries including Italy, the Netherlands, the US, Finland, Belgium, Canada, Hong Kong, and New Zealand

• Provides healthcare professionals with predictive reports and decision support tools tailored to each patient’s unique profile
